Gippsland Care Services provides dementia care using a person-centred approach. Plans of care are developed for clients focusing on enhancing their well-being and minimising troublesome behavioural and psychological symptoms. Education and support is also provided for families and carers, on request. The aim of care delivery is to improve the well-being of people living with dementia and assist carers and families to build positive care relationships with their loved ones.
Hospital to Home is a unique service providing transfer to home and short term health monitoring for people who have been discharged after an acute hospital admission. It includes assistance with discharge plans and face to face and telephone monitoring consistent with the client’s needs and preferences.
Alongside the eight ongoing Support at Home classifications, there are three short-term pathways to support temporary or specialised care needs:
Restorative Care Pathway: Short-term, goal-based care (up to 16 weeks, max two times a year) to help regain independence after illness, injury, or hospitalisation.
End-of-Life Pathway: Compassionate support at home for people with life-limiting conditions, available for one 12-week period.
Assistive Technology & Home Modifications (AT-HM) Scheme: Tiered funding over 12 months for equipment or home changes that enhance safety, accessibility, and independence.
